REVIEW: Treatment of ankylosing spondylitis and extra-articular manifestations in everyday rheumatology practice

A review published in the journal Rheumatology looks at the treatment of AS and its "extra-articular manifestations", such as when the disease affects the eyes, the gastrointestinal tract and/or the skin.

The authors found that although the efficacy of TNF-inhibitors such as Remicade, Humira and Enbrel (infliximab, adalimumab, etanercept, respectively), "appear to have similar efficacy" when treating ankylosing spondylitis, they seem to differ when treating the other "extra-articular manifestations".

They state that, "When evaluating as to which agent to use in the treatment of AS, an important consideration is the overall well-being of the patient. This should include any additional inflammatory burden that manifests in other parts of the body."

In the review, the go on to state that "Based on current evidence, among TNF inhibitors, the monoclonal antibodies (infliximab and adalimumab) are more appropriate than etanercept if extra-articular manifestations or comorbid conditions are present or suspected. To date, infliximab appears to be the best studied agent with a wide spectrum of proven efficacy."
